What We Do

As part of our commitment to the CFS program, Sentry Living Solutions will:

1. Identify and Provide Support:

• Assist individuals in successfully living in their family homes.

• Provide resources and tools to help individuals and families navigate services.

2. Deliver Tailored Assistance and Training:

• Educate families on the individual’s diagnosis and specific needs.

• Help individuals and families overcome barriers to accessing resources, including generic and regional center services.

3. Coordinate Services:

• Ensure consistency in training across providers.

• Schedule and coordinate medical appointments and other essential services.

• Identify transportation options or services.

4. Backup and Emergency Support:

• Identify backup providers and deliver emergency support if needed.

5. Futures Planning:

• Support families, including aging caregivers, in planning for the individual’s future.

• Provide training to maximize independence and ensure sustainability.

6. Community Engagement:

• Help individuals access community resources and participate in meaningful activities.

• Provide education to stakeholders, empowering individuals to engage fully in their communities.

7. Ongoing Monitoring and Reporting:

• Prepare and submit quarterly progress reports to ensure transparency and accountability.
